Here is a link to printing out your own 'Emergency go-kit Passport'. What it is is a booklet containing your Name, address, family, emergency contacts, medication, allergies, ect.

Let me tell you; when you are too sick (or injured) to speak for yourself, this is a nice thing to have around. I went to the E.R. Last year and kept slipping in and out of consciousness. Luckily, I gave the EMTs my Emergency booklet. When I woke up the next day, I was in a room and my Dad was there. They never figured out what was wrong, but the local hospital is useless. I don't drink, smoke, or do drugs. So what happened was a real mystery.

You can order actual mini booklets, sent to your door for free, but I think it's only for Oregonians. But you can still use the link to print out your own. It is recommended that each person in your family have their own copy.

Great information, thank you! One of my goals for this year is to make a proper emergency and evacuation kit. When planning for evacuation, don't forget your critters!

I keep all that info on my iPhone and I keep my husbands as well and he keeps mine. Came in real handy when we had to call 911 and needed my medication list.
